Pesticide Certification Program - Form Fees and Exams

Application forms - All require your signature
The following applications are not accepted via fax or email, as we require your original signature in ink:

Dispenser Application - This form is for Dispenser renewal, reciprocity, and replacement certificates,  Also if you currently hold a full, unrestricted Pesticide Applicator certificate and would like a dispenser certificate you can submit this completed form to our office for that.

Applicator Recertification - This form needs to be submitted to have your Applicator certificate(s) recertified. 

Applicator Reciprocity - If you have a valid certificate in another province and would like to have an Alberta certificate issued, you will need to submit this completed application to our office.

Applicator Replacement - If you require another copy of your certificate, you must submit this form to our office.

Special Certificate Grading - If you are submitting Lesson 505, 801 or 804 for grading, this form must be included with the Lesson or Lesson cannot be graded.

Name Change - If you have changed your name and would like us to update it on our system, you must submit this form along with the required documentation.  If you require a new certificate to be printed please submit the Applicator Replacement application as well.

  • Complete course packages include the Core Lesson (common to all certification classes) and a series of lessons that differ depending on the certification class.
  • Individual lessons can be ordered separately to add other certification classes to your qualifications.

Authorized assistant training check list
The Training Checklists must be completed with supervising certified applicators: Prior to any pesticide application; whenever the pesticide, application equipment or use location changes; and at least yearly afterwards.
Alberta Environment forms & resources
All of Alberta Environment's pesticide applicator related forms are available on the pesticide management application forms page. Those forms include:
  • Service Registration - This is required if you want For those applying pesticides as a business (commercial pesticide application)
  • Request for Credit Application Forms - course or event coordinators use to apply for pesticide applicator credits at a workshop/conference or a training event

Exams and proctored exam locations
What you need to know:
  • you book your exam by contacting the pesticides office in Vermilion at 1 866 853 8646
  • exams vary in length from 1.5 - 4.5 hours in length
  • all are closed book
  • once you pay the exam fee, you are issued a password and date for the proctored exam location of your choice
  • exams are available in an electronic format at a specified proctor centres throughout Alberta. Paper exams can be arranged.
  • are specific to the class(es) of applicator certification
  • you may challenge the exam without doing the homestudy course. Note that the lessons are specifically designed to help you pass the certification exam
  • 70%, or better, is the pass mark
  • you may apply for a rewrite if you get less than the passing grade
