Forms Fees and Exams
The listed forms can be filled out and returned either by:
- by mail to Pesticides Office Lakeland College 5704 College Drive Vermilion AB T9X 1K4
- by emailing a completed form. Since most forms require a signature you'll have to fill in, print, sign and then scan the form. The scanned form can be emailed to pesticides@lakelandcollege.ca
Also note:
- We work on a 15-business day turn around time on all applications.
- Payments can only be processed on Tuesday or Thursdays most weeks.

The following applications are accepted via email (pdf format only), signatures are required on the forms. Email to pesticides@lakelandcollege.ca Commercial Pesticide Dispense Application (pdf)- This form is for dispenser renewal, reciprocity, and replacement certificates, Also if you currently hold a full, unrestricted Pesticide Applicator Certificate and would like a dispenser certificate you can submit this completed form to our office for the additional certification. Applicator Recertification form (pdf) - This form needs to be submitted to have your Applicator certificate(s) re-certified. Applicator Reciprocity form (pdf) - If you have a valid certificate in another province and would like to have an Alberta certificate issued, you will need to submit this completed application to our office. Applicator Replacement form (pdf) - If you require another copy of your certificate, you must submit this form to our office. Request an Exam form (pdf) - To request an exam for any of our courses or certificates. Special Certificate Grading form (pdf) - If you are submitting Lesson 505, 801 or 804 for grading, this form must be included with the lesson. If it isn't included, the lesson can't be graded. Name Change form (pdf) - If you have changed your name and would like us to update it on our system, you must submit this form along with the required documentation. If you require a new certificate printed, please also submit the Applicator Replacement form. |

All of Alberta Environment's pesticide applicator related forms are available on the pesticide management application forms page. Those forms include:
Authorized assistant training checklist: The training checklist must be completed with supervising certified applicators: Prior to any pesticide application; whenever the pesticide, application equipment or use location changes; and at least yearly afterwards. The training checklist is also available in the Environmental Code of Practice for Pesticides.
